Please complete the Pre-Authorized debit/Automatic Bank Withdrawal Form and provide us with a copy of a void cheque. Please return the completed form to the Office in the Resurrection Parish.

Administration fees are applied to donations made through Canada Helps; a one-time donation is subject to a 4% fee, monthly donations are subject to a 3.5% fee. Tax receipts for donations from Canada Helps are issued by Canada Helps, not through the parishes.
